At Home Residential Habilitation (21 and over)At-Home Residential Habilitation provides services to Medicaid Waiver Enrolled individuals who reside with family members or independently in the community.
This service provides the opportunity for the individuals to further their skills in the areas of mobility, social behaviors, self care, basic safety, housekeeping, personal hygiene, health care, and financial management. This service is provided by a Residential Habilitation Worker who is assigned to the individual and works one - to - one on a regularly scheduled basis. An individual who wishes to receive At - Home Residential Habilitation is referred by a Medicaid Service Coordinator.
This life skills training program is performed primarily in the home of the individual being served. Opportunities to access the community can also be provided.
