This specialized seminar focuses on educating family members regarding the supports and services available in each of the organizations and share how a person can move seamlessly across the family of organizations.

Learning Objectives:

  • Differentiating Types of Residential and Day Programs and services across agencies: We will explore the various residential, day programs and other services that are available within our organizations. This will include a detailed examination of the differences between each program, helping participants understand the unique features and benefits of each option.
  • Explaining the Wide Array of Services: Our organizations offer a diverse range of services to support people with disabilities with varying needs. We will discuss these services in detail, highlighting how they contribute to the overall well-being and development of the people we serve.
  • Supporting people with disabilities through Changing Stages: It is crucial to recognize that a person’s needs may evolve over time. We will explain how our organizations can provide continuous support through each stage of an individual’s life, ensuring they receive the appropriate level of care as their circumstances change.
  • Assisting with Transitions to Different Levels of Care: Just as a typical person may need to move into different levels of care throughout their life, people we support may also require such transitions. We will discuss the processes and strategies we can use as an organization to assist with these transitions, ensuring they are smooth and beneficial for the individuals involved.

Presenters:

Sallyanne Burgess serves as the Vice President of Operations for Citizens Options Unlimited, bringing over three decades of dedicated experience to the organization. She began her career in 1989 in a Direct Support Role and has since held a variety of positions within the field, including Direct Support, Management, Healthcare, Management, Training and Quality Assurance. Prior to joining Citizens, Sallyanne spent ten years with OPWDD as a Standards Compliance Analyst, further deepening her expertise in standards and quality assurance.  Throughout her career, Sallyanne has demonstrated an unwavering commitment to the people supported by Citizens, as well as the Direct Support Professionals who make a difference every day. She is passionate about fostering a safe and nurturing environment, ensuring that every individual receives the highest quality of care and support.

Christopher O’Connor began his career more than 40 years ago as a Direct Support Professional with OPWDD. He later transitioned into a compliance role, auditing provider agencies and ensuring adherence to regulatory standards. After returning to school to earn his Registered Nursing degree, he moved into the provider sector. Christopher joined AHRC Nassau in 2007 and currently serves as Chief Operating Officer, overseeing all programs and services across the organization’s family of agencies. His career reflects a lifelong commitment to quality care, regulatory excellence, and person-centered services.
