This specialized seminar focuses on providing essential guidance and support for people with developmental disabilities and their families as they navigate life’s most significant transitional stages following high school. We will provide practical tools, emotional support, and specific resources to manage the move from High School to Adult Day Services and the crucial shift from Living at Home with Family to a Residential Setting.
Learning Objectives:
- Differentiate Service Models: Participants will be able to compare and contrast the key functional, legal, and structural differences between school-based supports (IEP/504) and adult-service systems (Life Plan/Waiver programs), identifying core changes in goals, funding, and advocacy roles.
- Evaluate Day Service Options: Participants will be able to identify and evaluate different models of adult Day Services (e.g., Supported Employment, Day Habilitation, Pre-Vocational Training, Community Hab) and articulate which service model is most appropriate based on an individual’s specific skills, interests, and desired outcome.
- Develop a Transition Checklist: Participants will be able to create a preliminary checklist for the transition from home to a residential setting that addresses both emotional preparedness, and practical logistics (e.g., OPWDD CRO (Certified Residential Opportunity) Process/Emergency Need list/organizing necessary documentation).
- Practice Self-Determination Techniques: Participants will be able to utilize specific communication techniques that promote and support the individual’s self-determination and meaningful choice-making in new environments (self-interest survey, POMS, etc.) such as during service selection or daily residential routine planning.
