This interactive workshop introduces attendees to Person-Centered Planning as a shared approach to helping people live lives that reflect their dreams, interests, and desires to achieve what matters most to them. Together participants will learn how person-centered thinking informs the Life Plan, and how the Personal Outcome Measures® (POM) tool identifies, tracks, and supports people to achieve meaningful goals.

Learning Objectives:

  • Learn How the POM Tool Supports Goals and Measures Progress. Gain an understanding of the purpose and structure of the Personal Outcome Measures®. Attendees will explore how POM outcomes align with Life Plan goals. Learn how POM information helps track progress and identify areas for improvement supporting people to live their own best lives.
  • Understand “What Matters Most,” for a Person to Inform their Life Plan. Learn to use person driven conversations to discover strengths, preferences, and priorities. Acknowledge how a meaningful life includes health, safety, and responsibility that are balanced with choice and control
  • Attendees will gain an understanding of Person-Centered Planning as the Foundation for Goal Achievement. Describe the core principles of person-centered planning. Explain how person-centered planning shifts focus from services to the person’s life. Recognize how families, self-advocates, and staff share responsibility in supporting goals
  • Use Person-Centered Planning, the Life Plan, and POM Together to Achieve Meaningful Outcomes. Practice connecting person-centered information to Life Plan goals and POM outcomes. Identify practical actions families, self-advocates, and staff can take to support success. Develop next steps for using these tools together in everyday life, services, and advocacy
